Immunizations/Physicals

State law requires that each student be properly immunized before attending school.  Parents must complete the Certificate of Immunization Status form, showing dates of required immunizations as listed below by grade.  If there are medical, religious or philosophical reasons why your student should not be immunized, you must sign a waiver and present it at the student's school.  For further information contact your family physician's office or our school nurse at (509) 432-4536.

 

Kindergarten

  • 4 doses DTP/DTaP provided the last dose is given on or after the 4th birthday
  • 3 doses Polio provided the last dose is given on or after the 4th birthday.  However, 4 doses are required if the same type of vaccine (IPV or OPV) is NOT used.
  • 2 doses Measles; 1 dose Mumps & 1 dose Rubella--both given on or after the 1st birthday and at least 28 days apart.
  • 3 doses Hepatitis B*

Grades 1-4:

  • 3 doses DTP/DTap/DT/Td provided the last dose is given on or after the 4th birthday
  • 3 doses Polio provided the last dose is given on or after the 4th birthday.  However, 4 doses are required if the same type of vaccine (IPV or OPV) is NOT used.
  • 2 doses Measles; 1 dose Mumps & 1 dose Rubella--both given on or after the 1st birthday and at least 28 days apart.
  • 3 doses Hepatitis B*

Grade 5:

  • 3 doses DTP/DTap/DT/Td provided the last dose is given on or after the 4th birthday

Grades 6-7: (for students new to the district)

Grades 8-12: (for returning students)

  • 3 doses DTP/DTap/DT/Td provided the last dose is given on or after the 4th birthday
  • 3 doses Polio provided the last dose is given on or after the 4th birthday.  However, 4 doses are required if the same type of vaccine (IPV or OPV) is NOT used.
  • 2 doses Measles; 1 dose Mumps & 1 dose Rubella--both given on or after the 1st birthday and at least 28 days apart.
  • Hepatitis B is not required.

* Hepatitis B recommendation is for:

  • 2nd dose to be given 1-3 months after 1st
  • 3rd dose to be given 2-6 months after 2nd, but
  • 3rd dose should not be given less than 4 monhts after the 1st, and
  • 3rd dose should not be given before 24 weeks of age.

Physicals...

School physicals are only required if your student will be playing a sport in grades 6-12.  Physicals must be done prior to the first day of practice.  You can receive sport physical forms from your school office.
